HOW CAN I ACCESS THE SUPPORT AT HOME PROGRAM

If you or your family are feeling they need a little support to keep a loved one living at home as they get older, then you could be eligible for Support at Home.

You will need an aged care assessment to access most Government-subsidised care including Support at Home.

This a free assessment that usually happens in your home and involves an assessor coming and speaking with you and your family about what areas of home life you might need support with.

Our team will also assess your level of need. There are eight levels in Support at Home:

WHAT CARE AND SUPPORT SERVICES ARE AVAILABLE:

The Aged Care Assessor will work with you and your representative/s to develop a Support Plan. The Support Plan will include your goals and services you have been approved for under the Support at Home service list. You will receive a ‘Notice of Decision’ with a copy of your Support Plan.  When you choose Linked to Life as your preferred provider, our Coordinator will develop your quarterly budget and help establish your approved services. Core services under Support at Home fall into three main categories:

WHAT WILL IT COST ME?

Our price list sets out the cost of all care and services we deliver.  You will pay a percentage of the hourly rates charged as set out below:

When you commence in the Support at Home program you will be required to contribute to the cost of certain services. The amount you will be required to pay is determined by the outcome of your income and assets assessment by Services Australia. Standard participant contribution rates from 1 November 2025

Clinical Care – 0%     Independence services – 5%   Everyday Living – 17.5%

Clinical Care – 0%         Independence services: Between 5% and 50% depending on income and assets            Everyday Living:  Between 17.5% and 80% depending on income and assets

Clinical Care – 0%        Independence services – 50%             Everyday Living – 80%

Lifetime cap:

There is a lifetime cap on contributions. Once you have paid the lifetime cap amount ($135,318.69 as at 20 September 2025) towards your services you will not be charged any more for the services you receive. The lifetime cap is indexed on 20 March and 20 September each year.
