If you or your family are feeling they need a little support to keep a loved one living at home as they get older, then you could be eligible for a Home Care Package.
You will need an aged care assessment (ACAT assessment) to access most Government-subsidised care including Home Care Packages.
This a free assessment that usually happens in your home and involves a team of healthcare professionals coming and speaking with you and your family about what areas of home life you might need support with.
Our team will also assess your level of need. There are four levels of Home Care Packages:
Linked to Life offers all four levels of Home Care Packages.
If you want help organising your assessment, feel free to call our office on 02 6651 7365 and we can assist you.
Or you can call My Aged Care Direct on 1800 200 422, or visit www.myagedcare.gov.au
At Linked to Life we respect your individuality.
You choose what care and support best suits not only your physical needs, but your lifestyle and overall wellbeing. Some of the services we can provide include:
Daily Care Fee:
If you are on a full Government pension, most of your Home Care Package will be subsidised by the Government, but you may need to pay a care fee toward your package if the level of care and supports you require exceeds the value of the Government subsidy paid to you. Any care fee will be negotiated and agreed to by you.
Income-Tested Care Fee:
Based on the outcome of your Services Australia income and assets test, the Government may impose an income-tested fee that will reduce the amount of government subsidy paid to you.
If you are required to pay an income-tested care fee, Services Australia will send you a letter advising of the daily amount you will need to pay. Your Coordinator will discuss payment options with you at the initial in-home assessment.
Any contributions/fees you pay add to the value of your home care package, increasing the overall income you have available to you for your care and services. Any unspent care or income-tested care fees paid by you will be refunded to you if you leave the package.
There are special circumstances where these fees can be negotiated if you are experiencing financial hardship. You can read more about home care fees on the My Aged Care website – www.myagedcare.gov.au
